Saúde mental: Novas configurações na intervenção do serviço social

Abstract

A presente tese de doutoramento em Serviço Social, analisa a intervenção dos assistentes sociais nas Equipas Comunitárias de Saúde Mental (ECSM) criadas pelo Despacho n.º 8455/2022, de 11 de julho. Desenvolvida no contexto pós-pandemia da Covid-19, a investigação incide sobre as dinâmicas profissionais, institucionais e socioculturais que moldam o trabalho destes profissionais num modelo de cuidados comunitários ainda em consolidação. A estratégia metodológica combinou métodos qualitativos e quantitativos, incluindo entrevistas semiestruturadas com profissionais de nove das dez equipas existentes, observação direta e análise quanti-qualitativa de segmentos de texto. Os principais resultados evidenciam o valor singular da intervenção social em três dimensões centrais. A primeira refere-se à promoção da recuperação psicossocial por via da inclusão comunitária, com efeitos na reintegração social, identidade e participação dos utentes; a segunda à contribuição para a redução de internamentos reincidentes e do recurso reiterado a urgências, por meio de alternativas sustentadas à institucionalização prolongada e de continuidade de cuidados; e a terceira, ao reforço de redes formais e informais de apoio, mobilizando famílias, comunidades e organizações para respostas mais holísticas e efetivas. Em contrapartida, a investigação identifica fragilidades estruturais que comprometem a eficácia e a autonomia técnica dos assistentes sociais: precariedade contratual, ausência de carreiras especializadas e de planos de progressão, escassez de recursos humanos (com poucos profissionais em regime de tempo inteiro), défice de formação contínua específica em saúde mental e falta de supervisão profissional. A hegemonia do modelo biomédico e a avaliação predominantemente quantitativa das práticas tendem a relegar a dimensão social e limitar a interdisciplinaridade. Por último, com este trabalho pretendeu-se contribuir para o fortalecimento do papel do Serviço Social na saúde mental, oferecendo uma visão abrangente sobre as novas configurações da prática profissional e promovendo a reflexão e o debate sobre a profissão.

This doctoral thesis in Social Work examines the intervention of social workers within the Community Mental Health Team establisheds by Ministerial Order No. 8455/2022 of 11 July. Developed in the post-Covid-19 context, the study focuses on the professional, institutional, and sociocultural dynamics that shape their practice within a community-based care model that is still in consolidation. The methodological strategy combined qualitative and quantitative methods, including semi-structured interviews with professionals from nine of the ten existing teams, direct observation, and a mixed quantitative-qualitative analysis of textual segments. The findings highlight the distinctive value of social work intervention in three core dimensions. The first concerns the promotion of psychosocial recovery through community inclusion, with positive effects on service users’ social reintegration, identity, and participation. The second relates to the contribution to reducing recurrent hospitalisations and repeated use of emergency services, by providing sustainable alternatives to long-term institutionalisation and ensuring continuity of care. The third dimension refers to the strengthening of formal and informal support networks, mobilising families, communities, and organisations towards more holistic and effective responses. Conversely, the research identifies structural fragilities that undermine the effectiveness and technical autonomy of social workers: contractual precariousness, lack of specialised career pathways and progression structures, scarcity of human resources (with few full-time professionals), insufficient continuous training in mental health, and the absence of professional supervision. The dominance of the biomedical model and the predominantly quantitative evaluation of practices tend to marginalise the social dimension and constrain interdisciplinarity. Finally, this thesis seeks to contribute to strengthening the role of Social Work in the field of mental health by offering a comprehensive understanding of the emerging configurations of professional practice and by promoting reflection and debate on the profession.
